Community-Based Care

What We Do

In district health offices across the county, we’re here to provide a variety of walk-in or “Nurse of the Day” health services:

  • immunizations for both children and adults
  • tuberculosis services (testing and medications)
  • blood pressure screening
  • general health counseling and referrals, and more

We make home visits, too. Anyone in the community can request us to visit a friend or family member, if they’re concerned about their well-being.

Prenatal care coordination is a common occasion for a home visit: showing up to provide the medical care necessary for a healthy mom and baby, during and after pregnancy — then following up to make sure everyone stays healthy.

In our offices or during home visits, we also regularly provide education to help prevent the spread of communicable diseases.
