What We Do
Healthy Families is a free, voluntary home visiting program — we believe families are best supported in their homes, to create a nurturing and healthy environment where children thrive.
We are here to provide resources, education and support: we begin our visits during pregnancy and continue visiting through a child’s third birthday. Our referrals come from a variety of sources. Anyone can call and make a referral, and we often receive self-referrals. Once a referral is received, a Family Resource Specialist (FRS) will arrange an appointment to meet with the family in their home or in the hospital — they arrive with a small gift and basic community resource information, and gather information to determine any additional services.
If the family meets further eligibility requirements, they may be offered ongoing home visits from Healthy Families: they’ll be assigned a Family Support Specialist who meets with them regularly in their home, providing information and working to set goals.
Also, the Family Support Specialist will bring lots of fun activities to promote parent/child bonding and stimulate the child’s growth and development. Also, they'll share information about healthy childhoods, safety information, parenting issues, and links to any other community resources.
All along the way, the Family Support Specialist will assist by monitoring immunizations and well baby visits, and by administering the ASQ Developmental screen at a variety of intervals.

- Be referred prenatally, or before your baby is 3 months old
- Meet income guidelines
- Not be enrolled in the following home visiting programs:
- Nurse Family Partnership
- WeCare
- Other Healthy Families
