What We Do
The Central Records department is responsible for maintaining, tracking and storing all MCPHD client records.
You can receive a copy of your MCPHD clinic records by contacting the Central Records department at 317.221.2379 or 317.221.2377 or going to the MCPHD clinic where you received services and completing a Release of Information form.
